Dear Applicant,

The transitional housing program of Keystone Opportunity Center (KOC) provides affordable, decent housing along with the support services needed for a family to move from homelessness to self-sufficiency, time limited month to month program.  KOC is committed to helping transitional households move from “dependency” toward “self-sufficiency.”

Households must be homeless to be eligible.  Our selection process has three basic steps:

  1. The enclosed Intake Worksheet and Consent returned to Keystone Opportunity Center.
  2. Applicants are interviewed.
  3. Our selection committee reviews all information from each applicant and selects one for the housing program.

Enclosed are a lengthy Intake Worksheet, other forms, and information.  Please carefully complete all information requested in order for your application to be considered.

  1. Intake Worksheet. Although long, you will find it is organized into separate categories Please make sure you have answered all questions.
  1. Consent to Release Information. Sign and date.
  1. Mail or bring completed Intake Worksheet and Consent, to the office.  Mailing address:  PO Box 64183, Souderton, PA  18964.  Street address: 104 Main Street, Souderton, PA.  If we do not receive your forms by ________________ we will assume you are not interested in applying for transitional housing.  Your application process begins when we receive these forms!
  1. Interview Scheduled. You will be called to schedule a personal interview with our case manager.  Bring with you to the interview all items on the enclosed Checklist.
  1. Check List for Interview. You will need to bring these items to your interview with the case manager.
  1. Service Agreement. For your information.  If you are selected, you will sign this Service Agreement as part of your lease.

Unfortunately, we cannot offer housing to every applicant, even though each one has an urgent housing need.  The selection committee considers every applicant carefully to determine needs and vulnerability.  Please know at time Keystone is constrained by lack of resources, the size of the vacant apartment, and other factors.  Keystone Opportunity Center is committed to a firm policy of non-discrimination.  Keystone provides equal opportunity to all applicants regardless of a person’s race, religion, color, sex, age, national origin, medical condition, material status, sexual orientation, veteran status, disability, or any other legally protected status.  There is an appeal process if you feel your application was not approved because of discrimination.

Please keep in mind it is not just affordable housing that Keystone Opportunity Center provides.  Because our goal is that you move from crisis to stability and self-sufficiency, you will be required to work hard.  Our goal is for you to be able to afford permanent housing as quickly as possible.
